Have an upcoming appointment at Gambrills Veterinary Center? Please complete the appropriate form below before your visit to help expedite your check-in process and ensure we have the information needed to provide the best possible care for your pet.

Exotic Boarding Admissions Form

1. In order to assure the health of your pet and other pets that are staying with us, we require proof that your pet has had a physical exam within the past year, is up to date on vaccinations, has had a negative intestinal parasite exam, and is free of external parasites. If your pet is not current on the health requirements we will arrange an appointment with our veterinarian at the time of admissions.    

Annual Health Needs or Requirements for Exotic Admission:

All Exotics: Negative Intestinal Parasite Fecal Exam 

Guinea Pigs: Bordetella Vaccine 

Ferrets: Rabies and Distemper Vaccines 

       

2. We are open Monday through Friday from 6:30am to 6pm and Sundays from 10am to 6pm for drop off and pick up of your pets. The latest drop off or pick up 6pm Sunday - Friday. If you are dropping off or picking up after 6pm, a late fee of $15 will be charged. Picking up after 12pm Monday - Friday will result in a half day charge.


I understand that Gambrills Veterinary Center does not provide 24 hour care.

3. Your pet will be fed according to the feeding instructions you provide. We DO NOT provide food for exotic animals so please bring enough of his/her food for the stay. Make sure all foods and treats are labeled at drop off. Please package individual meals in Ziploc bags or containers for each meal during your pet's stay. 


4. Bring all medications  in original containers, instructions will be recorded at drop off. There is an additional $30/day fee for medicated boarding. If medications are not provided with a veterinarian-prescribed label, we CANNOT administer them to your pet to ensure their safety and compliance with proper medical standards.


5. Please leave your regular cage/enclosure, and caretaking supplies with us. This provides your pet with a more comfortable stay and allows us to care for your pet in the same manner as you would at home.


6. We strive to provide the best care & compassion for your pets. GVC uses all reasonable precautions against injury, escape, or demise.  However, occassionally special or emergency treatments are needed while your pet is in our care. If this occurs, we will attempt to contact you or your designated contact to inform them of the situation. If we are unable to contact you, it is agreed that GVC & staff is authorized to provide veterinary services, as required, or in emergency circumstances to follow through with such procedures as are necessary for the well-being of your pet on a continuing basis until further advised in writing.


7. If any pet(s) is not picked up within three (3) days of the specified release date, a written notice will be mailed to the address above. Five (5) days after such notice is mailed, the animal will be considered abandoned and may be disposed of as the clinic deems appropriate. It is understood that abandonment does not relieve me from the responsibility of payment for services rendered, including the cost of boarding.


I understand that I assume financial responsbility for all services rendered.  An estimate of fees will be provided upon request. A deposit is required to schedule boarding at Gambrills Veterinary Center. The deposit will be applied to the final invoice. Deposits are completely refundable with notice greater than 24 hours. However, same day cancellations or no-shows will forfeit the deposit which must be paid again for future boarding. All charges shall be paid in full upon pet's release. In the event payment is not received for outstanding balances related to services rendered prior to the pet's discharge and no verbal payment arrangements have been established, the clinic reserves the right to charge the card previously used or the card on file. I agree that in the case of nonpayment, a fee of 1.5% per month (18% per annum) will be charged. All reasonable collection and attorney fees necessary to collect this debt will be borne by me.

Canine/Feline Boarding Admissions Form

If your pet is not current on the health requirements we will arrange an appointment with our veterinarian at the time of admissions.

If your pet is on a special diet or is a finicky eater, please bring enough of his/her food for the stay. Make sure all foods and treats are labeled at drop off. Please package individual meals in Ziploc bags or containers for each meal during your pet's stay. There is no additional charge based on what food is fed. 


4. Medications: Bring all medications with you to drop off with your pet in original container, instructions will be recorded at drop off. There is an additional $30/day fee for medicated boarding. If medications are not provided with a veterinarian-prescribed label, we CANNOT administer them to your pet to ensure their safety and compliance with proper medical standards.

During summer months: All boarding pets will be administered Capstar, an oral flea preventative, prior to admission free of charge.

Doggie Daycare First Day Admissions Form

1. In order to assure the health of your pet and other pets that are staying with us, we require proof that your pet has had a physical exam within the past year, is up to date on vaccinations, has had a negative intestinal parasite exam, and is free of external parasites. If your pet is not current on the health requirements we will arrange an appointment with our veterinarian at the time of admissions.

Tests: Negative Heartworm Test, Negative Fecal Intestinal Parasite Exam           


2. We are open Monday through Friday from 6:30am to 6pm for drop off and pick up of your pets. We are closed on Saturday, and Sunday you can drop of or pick up from boarding but there is no daycare. Picking up after 6pm Monday - Friday will result in an additional $10 late fee. If we cannot get in contact with you by 6:30pm   and your pet is still in daycare, they will be placed in boarding and charged the regular boarding fee for the night.

3. Your pet will not be fed during daycare unless you bring food with you and verbally provide feeding instructions. Make sure all foods and treats are labeled at drop off. If your pet is placed in the kennel overnight, we use Purina EN (a premium diet designed to prevent gastroenteritis) for feeding the kennel.


4. If your pet requires medication(s), please bring them in the original container and instructions will be recorded at drop off. There is an additional $5 fee for medicated daycare.

Written signature grants Gambrills Veterinary Center permission to use photos of your pet on our affiliated social media platforms. If you prefer your pet not to be shown on such platforms, please submit in writing your refusal and  such will be documented.

Grooming Release Form

In order to assure the health of your pet and other pets that are staying with us, we require proof that your pet has had a physical exam within the past year, is up to date on vaccinations, has had a negative intestinal parasite exam, and is free of external parasites. If your pet is not current on the health requirements we will arrange an appointment with our veterinarian at the time of admissions.

Your pet is important to us. Because we care, we want to assure you that every effort will be made to make your pet's visit as pleasant as possible. Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary.   


I understand that I assume financial responsibility for all services rendered. An estimate of fees will be provided upon request. A deposit is required to schedule grooming at Gambrills Veterinary Center. The deposit will be applied to the final invoice. Deposits are completely refundable with notice greater than 24 hours. However, same day cancellations or no-shows will forfeit the deposit which must be paid again for future grooming. All charges shall be paid in full upon pet's release. In the event payment is not received for outstanding balances related to services rendered prior to the pet's discharge and no verbal payment arrangements have been established, the clinic reserves the right to charge the card previously used or the card on file. I agree that in the case of nonpayment, a fee of 1.5% per month (18% per annum) will be charged. All reasonable collection and attorney fees necessary to collect this debt will be borne by me. I hereby grant permission to the Grooming establishment of Gambrills Veterinary Center to groom my pet, and if necessary, to obtain emergency veterinary treatment for my pet.   

Hospital Admissions Form

As owner, or duly authorized agent of the owner, of the above named animal, I hereby consent and authorize  Gambrills Veterinary Center to receive, prescribe, treat or operate on this animal.

I understand that during the performance of this procedure(s), unforeseen conditions may be revealed that necessitate an extension or variance in the procedure(s) set forth above. I expect Gambrills Veterinary Center to use reasonable care and judgement in performing the procedure(s).The nature of such services has been described to my satisfaction and I realize that no guarantee nor warranty can ethically or professionally be made regarding the results. I acknowledge that risks and the possibility of complications exist in any surgical or medical treatment.


I also authorize Gambrills Veterinary Center and staff to provide veterinary service as required or in emergency circumstances to follow through with such procedures as are necessary for the well-being of my pet on a continuing basis until further advised in writing.

Deposits are completely refundable with notice greater than 24 hours. However, same day cancellations or noshows will forfeit the deposit which must be paid again for future appointments. 


I understand that I assume financial responsibility for all services rendered. An estimate of anticipated fees has or will be given to me upon request. All charges shall be paid in full upon release. In the event payment is not received for outstanding balances related to services rendered prior to the pet's discharge and no verbal payment arrangements have been established, the clinic reserves the right to charge the card previously used or the card on file.

ALL ANIMALS ADMITTED MUST BE CURRENT ON THEIR VACCINATIONS AND MUST BE FREE OF EXTERNAL PARASITES. ANY ANIMAL FOUND TO HAVE FLEAS OR TICKS WILL BE TREATED AT THE OWNER'S EXPENSE.
